    Temporary library card application

    This library card application is a for a temporary library card. The applicant must come to the library and present photo identification (ID) to receive a physical library card in order to prevent the card from expiring. If the photo ID does not have a current address, then the applicant must show one additional item from the following list: valid voter registration card, valid current government issued ID (passport, military ID), recent bank Statement, recent utility bill, or tax receipts. Registration information is held confidential, pursuant to the laws of Indiana, for library purposes only, and may not be divulged or released without a court order.
    If you are unsure which township you live in, use the township map below to find out. Wayne and Orange townships are served by the Kendallville Public Library and Perry township is served by the Ligonier Public Library. If you live in one of these townships you will need to contact the library in your serving area to ask about getting a card.
    My checking below indicates that I agree to abide by library lending rules and all policies and regulations; that I agree to be responsible for overdue, lost, or damaged materials borrowed with this card; and, I understand that ALL library card holders in good standing will have access to the Internet and will be responsible for reading and accepting the “Computer and Internet Acceptable Use Policy” before accessing the Internet through library resources.
